Fakat bunun i\u00e7in \u00f6nceden ailelerden izin al\u0131nmas\u0131 geekiyor. \u00c7ocu\u011funun s\u00fct i\u00e7mesi y\u00f6n\u00fcnde izin veren ailelerin \u00e7ocuklar\u0131na MEB \u00fccretsiz s\u00fct da\u011f\u0131t\u0131yor. Ailenin izin vermedi\u011fi durumlarda ise \u00f6\u011frenci s\u00fct i\u00e7emiyor. Bu durum i\u00e7in de MEB 2016 \u00d6kul S\u00fct\u00fc izin formunu doldurup, velilerin \u00f6\u011fretmenlere ve okul m\u00fcd\u00fcrl\u00fcklerine vermesi gerekiyor. Okul s\u00fct\u00fc formu doldurmayan ailelerin \u00e7ocuklar\u0131na okul s\u00fct\u00fc malesef verilemiyor. Hal b\u00f6yle olunca da okul s\u00fct\u00fc veli izin formu 2016 en \u00e7ok aranan belgelerden bir tanesi haline geldi Mebpersonel.com olarak 2016 y\u0131l\u0131 MEB okul s\u00fct\u00fc veli izin formunu sizler i\u00e7in ara\u015ft\u0131rd\u0131k ve bulduk. A\u015fa\u011f\u0131da yer alan okul s\u00fct\u00fc izin formu ile \u00f6\u011frencinizin, \u00e7ocuklar\u0131n\u0131n\u0131z\u0131 \u00fccretsiz okul s\u00fct\u00fc kampanyas\u0131ndan faydalanmas\u0131na katk\u0131da bulunabilirsiniz. A\u015fa\u011f\u0131daki form doldurularak s\u0131n\u0131f \u00f6\u011fretmenine veya okul idaresine verilmesi gerekmektedir. Da\u011f\u0131t\u0131lan okul s\u00fctlerine ait analiz sonu\u00e7lar\u0131n\u0131 muhafaza etmek,<br \/>\n5. \u0130l Okul S\u00fct\u00fc Komisyonuna teknik destek vermek,<br \/>\n6. \u0130llerde, programla ilgili e\u011fitim \u00e7al\u0131\u015fmalar\u0131nda g\u00f6rev almakt\u0131r.<br \/>\n\u0130l Milli E\u011fitim M\u00fcd\u00fcrl\u00fckleri<br \/>\n1. \u0130l Okul S\u00fct\u00fc Komisyonunun sekretaryas\u0131n\u0131 y\u00fcr\u00fctmek,<br \/>\n2. Gazete\u2019de yay\u0131mlanan G\u0131da \u0130\u015fletmelerinin Kay\u0131t<br \/>\nve Onay \u0130\u015flemlerine Dair Y\u00f6netmelik kapsam\u0131nda onay belgesi veya bu Y\u00f6-<br \/>\nnetmelik h\u00fck\u00fcmlerine g\u00f6re ge\u00e7erli e\u015fde\u011fer belgeye sahip ve \u00fclke i\u00e7inde<br \/>\nUHT i\u00e7me s\u00fct\u00fc \u00fcretimi yapan g\u0131da i\u015fletmelerinden 4\/1\/2002 komisyonu<br \/>\nolarak g\u00f6rev yaparlar.<br \/>\n(6) \u0130llerde okul s\u00fct\u00fc komisyonlar\u0131n\u0131n sekretaryas\u0131n\u0131n y\u00fcr\u00fct\u00fclmesi, okul Bakanl\u0131\u011f\u0131nca sa\u011flan\u0131r.<br \/>\n(9) \u00d6\u011frenci velileri, \u00f6\u011fretmenler, aile hekimleri ve\/veya sa\u011fl\u0131k kurumlar\u0131nca<br \/>\ns\u00fcte kar\u015f\u0131 duyarl\u0131l\u0131\u011f\u0131 tespit edilen \u00f6\u011frenciler, okul y\u00f6netimleri taraf\u0131ndan<br \/>\nprogram d\u0131\u015f\u0131nda tutulur.<br \/>\n(10) Programda uzun d\u00f6nemde \u00f6\u011frencilerin geli\u015fmelerine ili\u015fkin \u00e7al\u0131\u015fmalar<br \/>\nMilli E\u011fitim Bakanl\u0131\u011f\u0131 ile i\u015fbirli\u011fi i\u00e7inde Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131nca y\u00fcr\u00fct\u00fcl\u00fcr.<br \/>\n(11) Da\u011f\u0131t\u0131lacak okul s\u00fctlerinin \u00fcretiminden t\u00fcketimine kadarki a\u015famalar\u0131nda,<br \/>\nT\u00fcrk g\u0131da mevzuat\u0131 ile ihale teknik \u015fartnamesine uygunlu\u011funun denetimi,<br \/>\nil g\u0131da tar\u0131m ve hayvanc\u0131l\u0131k m\u00fcd\u00fcrl\u00fcklerince yap\u0131l\u0131r.<br \/>\n(12) Okul s\u00fct\u00fc \u00fcretimi yap\u0131lan illerde, Bakanl\u0131k il m\u00fcd\u00fcrl\u00fcklerince s\u00fctlerin her<br \/>\nbir partisinden numune al\u0131n\u0131r, numunelere ait analiz sonu\u00e7lar\u0131, Bakanl\u0131\u011fa<br \/>\nve teslimi yap\u0131lacak illerdeki il g\u0131da tar\u0131m ve hayvanc\u0131l\u0131k m\u00fcd\u00fcrl\u00fc\u011f\u00fcne g\u00f6nderilir.<br \/>\nE\u011fitim ve tan\u0131t\u0131m<br \/>\nMADDE 5 \u2013<br \/>\n(1) Program\u0131n kamuoyunda tan\u0131t\u0131m\u0131, ileti\u015fim stratejisinin belirlenmesi ve programda<br \/>\ng\u00f6rev alan ki\u015filerin e\u011fitimleri i\u00e7in Bakanl\u0131k, Milli E\u011fitim Bakanl\u0131\u011f\u0131,<br \/>\nSa\u011fl\u0131k Bakanl\u0131\u011f\u0131 ve Ulusal S\u00fct Konseyinin kat\u0131l\u0131m\u0131yla gerekli tedbirler al\u0131n\u0131r.<br \/>\n(2) Program ba\u015flamadan \u00f6nce ailelere s\u00fct i\u00e7imi sonras\u0131 olu\u015fabilecek basit rahats\u0131zl\u0131klar<br \/>\nve bulgular\u0131n anlat\u0131ld\u0131\u011f\u0131, bu bulgular\u0131n b\u00fcy\u00fck bir b\u00f6l\u00fcm\u00fcn\u00fcn<br \/>\nge\u00e7ici ve hafif oldu\u011funun belirtildi\u011fi, s\u00fct\u00fcn \u00f6neminin vurguland\u0131\u011f\u0131 e\u011fitim<br \/>\nprogramlar\u0131 Milli E\u011fitim Bakanl\u0131\u011f\u0131 ve \u00f6\u011fretmenler<br \/>\ntaraf\u0131ndan yap\u0131l\u0131r.<br \/>\n(4) Program i\u00e7in gerekli e\u011fitim ve tan\u0131t\u0131m materyallerinin temini, yay\u0131m\u0131 ve da-<br \/>\n\u011f\u0131t\u0131m\u0131 Bakanl\u0131k koordinasyonunda, Ulusal S\u00fct Konseyi taraf\u0131ndan yap\u0131l\u0131r.<br \/>\n\u00dc\u00c7\u00dcNC\u00dc B\u00d6L\u00dcM<br \/>\nFinansman,
